Cloverbud Waiver of Liability
Cloverbuds are non-competitive 4-H members, ages 5-7. They are non-official members and therefore are not covered under the county’s 4-H insurance policy. Please fill out this form for the event or activity that your Cloverbud will be participating in. For Achievement Days, only one form needs to be submitted for all events, but please list each event your child will be attending (Statics, Livestock- baby beef, pig, sheep etc).
