•Planning for My Project Adventure (PA093) (fillable form) For members 8-10 years old. In order to complete the 4-H year, members 8-10 must complete at least one PA093; however, they may fill out one in each project area that they wish to be considered for an award.

•ND 4-H Project Plan (PA095)  (fillable form) For members age 11-19 years old.  In order to complete the 4-H year, members 11-19 must complete at least one PA095; however, they may fill out one in each project area that they wish to be considered for an award.

•ND 4-H Plan of Action (PA096) (fillable form) Members age 11-19 years old must complete this form in order to complete the 4-H year.
