4-H Upper Missouri Valley Fair Forms & Information

You may  print the appropriate entry forms for the Upper Missouri Valley Fair, complete and mail them to the Extension Office prior to June 1.  

If you wish to type data into these forms, first, be sure to save the fillable form to your computer and then complete and save it.   Mail the completed form to us (NDSU Extension/Williams County, PO Box 1109, Williston, ND 58802-1109) OR if you want to fax it, that will work too. Fax# 701-577-4597.  Completed forms may also be emailed to mindys@co.williams.nd.us however, please check your email for acknowledgement of receipt from us.
