- Form to apply for award discs.

Emmons County Banner Club Form
- Club leader fills out form to meet criteria to receive banner club award.

Member Enrollment Form
- A form members need to complete and return to Extension Office to be enrolled as a member.

4-H Participant Expectations/Media Release Form
- A form members need to complete and return along with their member enrollment form to the Extension Office. In doing so members agree to the expectations of them at a 4-H setting and they also give permission for their picture of themselves to be used with the media.

4-H Health Form
- A form members complete and return with the member enrollment form to the Extension Office with contact info if an illness should occur.

Leader Enrollment Form
- A form leaders fill out following youth protection clearance which registers them as a 4-H leader.

Emmons County 4-H Scholarship Form
- The 4-H scholarship in the amount of $200 is designed to assist current and former Emmons County 4-H members in their continued education beyond high school.

Emmons County 4-H Spring Awards Application Form
- Members 13 and over can apply for 2 award trips: Extension Youth Conference and Range Camp.

- Register exhibits to enter at Emmons County 4-H Achievement Days.

Absentee Interview Judging Form for Static Exhibits
- A form members can fill out and attach to their project to have judged if they are unable to physically attend Achievement Days.

Absentee Interview Judging Form for Livestock Exhibits
- A form members can fill out and turn in to the Extension Office prior to Achievement Days if they are unable to attend the Achievement Day livestock show which would allow someone else to show their animal.

Do Your Own Thing Form
- This project is designed to explore a topic of their own design. Usually the project selected is one that is not included in existing project material in the North Dakota 4-H program. The Do Your Own Thing form needs to be completed and accompany the project when being exhibited.

Individual Membership Report Forms
- Individual Membership participation is designed for youth who because of special circumstance cannot participate in the traditional club program. Five separate reports are required prior to Emmons County Achievement Days to be eligible to exhibit.

4-H Youth Interest Survey
- Form to be completed if someone is interested in joining 4-H and to be sent to the Emmons County Extension Office.
